What should I bring to my first appointment/walk-in visit?

You will need to bring the following with you to your first visit to our clinic:

Valid Louisiana ID or driver’s license
Proof of address
Proof of income need to specify
Social security card
Insurance card
Children seeking services are required to bring additional information. Please contact our Care Center at (504) 568-3130 for those items.
